Scope of service: This service plan includes but is not limited to the following components:
1. Assessment and evaluation of individual needs and goals.
2. Development of a personalized care plan tailored to the individual’s specific needs and preferences.
3. Coordination of services and resources to support the individual in achieving their goals
4. Regular monitoring and evaluation of progress towards goals.
5. Collaboration with other providers and professionals involved in the individual’s care.
6. Advocacy for the individual to ensure their needs are met and their rights are respected.
7. Ongoing support and guidance to help the individual navigate challenges and setbacks.
8. Continual reassessment and adjustment of the service plan as needed to ensure the individual’s needs are being met effectively overall, this service plan is designed to provide holistic and person-centered care to individuals in need of support services, with the ultimate goal of enhancing their quality of life and promoting their independence and well-being.

The provider service plan will be available for review by persons served, their family, significant others and the public.

We will revise and update the service description when any of the information is changes.
