This service can support people as they transition from living in a facility to living in a house or an apartment, with others or on their own. It can pay for one-time household expenses like the security deposit for an apartment or start-up fees for utilities.
Community Transition can also be used to buy initial cleaning products or household supplies.
When to Apply this Service
The person’s service and support administrator, or SSA, will assess them for a need for Community Transition purchases. If there is a need, the SSA will include it in the person’s individual service plan.
Community Transition may be authorized up to 180 calendar days prior to enrollment in the Individual Options Waiver and for up to 30 days after enrollment. Community Transition services cannot exceed $2,000 per person. Pre-transportation services covered as Community Transition cannot exceed $500 of the available $2,000.
Providing this Service
Agency providers that have a Medicaid provider agreement and are DODD-certified can provide this service. County boards of developmental disabilities can provide this service only when no other qualified provider is available.
The provider needs to maintain receipts of purchases.
Community Transition cannot be used for ongoing expenses, tobacco or alcohol, or items for entertainment or recreational purposes.
Training Requirements for this Service
Providers must complete the initial provider certification and annual training requirements to deliver this service.
Community Transition does not require additional training. A provider can add this service to an existing contract.
Billing and Payment Information
All services must be delivered as specified in the individual service plan and authorized in Payment Authorization for Waiver Services, known as PAWS, to be successfully submitted for payment through eMBS.
